Referrals:
Diverse Occupational Therapy works with people from 16 years
Located in Southern Adelaide, Fleurieu and Hills suburbs
Self, parent, carer, GP or other health provider referrals are accepted. All referrals are assessed individually.
Appointment times:
Initial session: 90 minutes
Subsequent sessions: 60 minutes
Appointment costs:
Private Health and Medicare
$193.99 per hour or part thereof + travel fee
Contact your health fund to determine if you are eligible and if any rebates may apply. Out-of-pocket costs apply.
Contact your GP to discuss if you are eligible for Medicare rebates as Occupational Therapy can be provided through a GP Management or a Team Care Plan.
NDIS funds
$193.99 per hour or part thereof + travel fee
Contact your NDIS support coordinator to determine which services may be eligible. Out-of-pocket costs may apply.
Services include face-to-face, travel, report writing, resource development, assessments, stakeholder communication and advocacy.
Travel fees:
Charged per visit, based on distance from Noarlunga Hospital:
$25 - 20km
$50 - 40km
$100 - 60km
Cancellations: Full charges apply for cancellations within 48hrs of appointment time. See our Cancellation Policy for more information.
Important:
Initial sessions are charged at a minimum of 1.5 hours, 75 minutes consultation and subsequent sessions are charged at a minimum of 1 hour, 50 minutes consultation. The remaining time is for documentation and follow-up.
All sessions are payable at the completion of the session. A receipt will be provided allowing you to claim from your Private Health, Medicare or NDIS fund for applicable rebates.
In the case of overdue charges, all future appointments will be cancelled until payment is received.
Some assessments incur additional costs, which will be billed directly to you. These will not be charged without your prior consent.
